Converge Behavioral Health, PLLC is a covered entity under HIPAA. We use and disclose your health information only for treatment, payment and practice operations, or as you authorize in writing, or as the law requires. Substance use treatment records receive the additional protections of 42 CFR Part 2 and are not shared without your specific written consent except in narrow emergencies. You have the right to see and get a copy of your record, to request corrections, to receive an accounting of disclosures, and to file a complaint with us or with the U.S. Department of Health and Human Services without retaliation. The full notice is provided at intake and available on request.
Sessions are delivered over a HIPAA-compliant video platform. Telehealth has limits: technology can fail, and some situations require in-person care. Your clinician will discuss a backup plan for interruptions and an emergency plan that includes your physical location at the time of each session. You may withdraw consent for telehealth at any time.
Converge does not discriminate on the basis of race, color, national origin, religion, sex, sexual orientation, gender identity, age, disability, or veteran status. Language assistance and accommodations are available at no cost; ask when you schedule.
Under the No Surprises Act, clients who are uninsured or self-pay have the right to a written Good Faith Estimate of expected charges before scheduled services. Ask for one at any time.
